
If you’ve ever searched for free tattoo consent forms online, chances are you’ve clicked a few promising links only to find out the forms aren’t actually free!
You can fill out these forms, but when you try to print or export them, you’ll hit a paywall.
Some websites do offer free templates, but they are static PDFs, meaning you can’t edit them.
To solve that problem, I’ve created a tattoo consent form template in Google Docs that you can edit as needed.
This template is completely free, and you don’t have to sign up. Simply click on “Get this template” and then “USE TEMPLATE,” and you’re good to go!
I’ve made two versions: a black and white one, which looks professional, and a colored one, which is more modern-looking.
You can completely customize the form to your liking—change the font, color, add your logo, and so on.

Tattoo Consent Form (B&W)



This is a 3-page comprehensive tattoo consent form designed to help tattoo studios, independent artists, and body art professionals collect informed consent from their clients.
It is professional, and we’ve organized everything using tables.
It covers all the essential aspects that an ideal tattoo consent form must have.
As mentioned earlier, it is fully editable; you can add more content to these tables or delete unnecessary tables, if needed.
This template includes:
- Client Information Section: Enter the full name, contact details, and address of the client.
- Emergency Contact Details: Space to enter the client’s emergency contact details, which is useful in case of an emergency.
- Tattoo Information: In this section, you can enter the specifics of the tattoo design, size, body placement, and the artist responsible.
- Medical History Checklist: Here, the client can disclose any relevant health conditions or allergies that may affect the procedure or healing.
- Consent and Acknowledgment Statements: Includes key legal disclosures about age, risks, permanence of tattoos, liability release, and aftercare responsibilities.
- Client Signature Section: For clients to sign and date, confirming understanding and agreement to the procedure and terms.
- Parental/Guardian Consent Section (for minors): Legally required section for underage clients, with parent or guardian’s signature and date fields.
- Tattoo Artist Certification: Signed by the tattoo artist to confirm the procedure was explained and all questions were answered.
- Optional Media Release Consent: Allows the client to approve or deny the use of tattoo images for promotional purposes.
Tattoo Consent Form (Color)



This form has the exact same content as the above form; it’s just that we’ve added color.
We’ve changed the font color, table border color, and table header colors to give it a more modern look. These colors are completely changeable.
You can even add your logo to the header; simply double-click inside the page header, and then drag and drop your logo image.
