How to Use These Templates
To use the templates, first, make sure you’re signed in to your Google account.
Then, click on the “Get this template” button. You’ll be directed to a new tab where you can preview the template.
On the top right of the page, you’ll see the “USE TEMPLATE” button. Simply click on it, and you’ll have your own copy ready to edit.
Free Consent Form Template for Botox (B&W)
This is a professional consent form template that’s designed to help practitioners collect all the essential information from the client before the procedure begins.
It also helps communicate the procedure details and obtain proper legal consent from the client. We’ve used tables to organize all the data to make the form look professional and neat.
We’ve also made a colored version of this form; we’ve changed the font color, table border, and header colors. You can also add your logo in the header; simply double-click inside the header and then drag and drop the image file.
