How to Use the Template
Simply select the template you want to use, click on the ‘Get this template’ button, and then click on the ‘USE TEMPLATE’ button to make a copy for yourself.
Please note that these templates are designed in Google Docs, so you’ll need to log in to your Google account before making a copy.
After copying, you can edit as needed and fill it out. You can then print it. If you prefer, you can also print the form as is and fill it out by hand.
Free Fake Doctor’s Note Templates
Fake Doctor’s Note Template Design-1
This template includes the medical provider’s information, including the name, address, phone number, and website.
The entire template is formatted using the default Arial font for a clean, professional look. Below the provider’s details, there is space to add the date, the patient’s name, the reason for their absence, and the specific date covered.
At the bottom, I have included a sample signature in the Qwitcher Grypen font, which mimics a handwritten style. Beneath the signature, the doctor’s name and address are provided.

This template is similar to the previous one, but I have added checkboxes to specify whether the patient’s absence is from work, school, or another activity, along with a reason for the absence (illness, injury, or other).
These checkboxes are clickable, so you can simply click on a box to check it.

This template is more of a letter and is longer than the previous templates.
I have bolded key information, such as the patient’s name, illness, and dates, to make them stand out. You can also choose to underline these details if you prefer.

This doctor’s note template is exactly like Design 3, except that I have added a watermark to make it look more realistic.
You can also add a watermark to other templates by going to Insert > Watermark, then adding the desired watermark image or typing text into the text field if you prefer a text watermark instead of an image.

And finally, the last template on the list. This template is super simple, and I have designed it to be minimalistic.
I intentionally left longer spaces after each sentence to give it a clean, minimal look. If you prefer smaller spaces, you can select the entire text, go to Format > Line & Paragraph Spacing, and choose the Single option.
