Are you thinking of adopting a dog or any other pet?
Then having the right paperwork is crucial.
If you search on Google for free dog adoption templates, you’ll find tons of options, but the problem is that most are in PDF format, which means you can’t modify the document at all. You can only print the document as is and then fill it out by hand.
Some websites provide forms that you can fill out online, but when you try to print or share the completed form, you’re often asked to create an account or purchase a subscription.
That’s why I’ve created these templates in Google Docs. This means you can edit the document as you like—whether you want to add details, change the font, adjust the paper size, alter the background color, or make any other modifications. Best of all, they are 100% free.
How to Use the Templates
To use the template, simply click the “Get This Template” button, which will take you to the template preview page. In the top right corner, you’ll see the “USE TEMPLATE” button—just click on it, and you can start editing the document as needed.
The document will be saved in your Google Drive. You can print the document and fill it out by hand, or add the details online, then print it or save it as a PDF to share online.
Dog Adoption Agreement Template Template #1
This is a simple one-page pet adoption template.
It covers all the essential details of the adoption process to ensure clarity and responsibility for both parties. I have created three tables: in the first table, you can enter the current owner’s details; in the second table, you can enter the pet’s details; and in the third table, you can enter the adopter’s details.
I have also listed some terms, which you can edit or add to as needed. At the end of the document, there is a section for the signatures of both parties involved.
Pet Rehoming Agreement Template Template #2
This is a pet rehoming agreement template that can be used for any pet you decide to adopt. The template begins by clearly mentioning the pet’s name, the name of the rehoming family, and the adopting family.
I have added empty lines where you can enter the details. If you are editing this template online, I suggest deleting these empty lines and then entering the details.
You can then select the entered details and press Ctrl + U or Cmd + U to underline them.
Next, I’ve included a table where all the pet’s details can be filled in. Finally, there are sections where both parties can enter their full name, signature, and date.
Dog Adoption Agreement Template Template #3
This is a different template. I generally use tables to organize all the details and keep the table colors grey to maintain a formal look.
However, in this template, I have colored the tables and added some additional information. I decided to go with a golden yellow color, which matches my dog Muffin (she is a 4-year-old Golden Retriever). You can change the color or font style as needed.

Dog Adoption Agreement Template Template #4
With this template, I decided not to use tables, but the details remain the same. Each section is divided by a horizontal line.
It begins with space for the agreement date, followed by the adopter’s information, the dog’s information, and editable terms and conditions.
Then comes the adoption fee section, waiver and liability, signatures, a small adoption checklist, and finally a short section for additional notes. The checklist boxes in the adoption checklist are clickable—simply click on the box to check it.
Alternatively, you can print the form and check the boxes by hand.
