How to use the Form
To edit the form, you need to make a copy for yourself. Since these forms are created in Google Docs, you’ll need a Google account first.
You can click on the “Get this template” button to go to the template page, then click on the “USE TEMPLATE” button located at the top right of the page to make a copy.
From there, you can edit the form as needed. You can also print the form as-is and fill it out by hand.
Direct Deposit Authorization Form Design-1
This form is a simple one-page document that includes all the essential details, such as employee information, bank information, deposit instructions, and authorization (signature and date).
There is also a small note at the end asking the employee to attach a voided check. The checkboxes in this form are not clickable.
If you are filling out the form online, to check a box, simply place the cursor where you want to insert a checked box, then go to Insert > Special characters. In the pop-up window, search for “Ballot” on the right panel, and from the left panel, select the checked box to insert it into the form.

This form is a standard Direct Deposit Authorization Form commonly used in the U.S.
It includes employee information, account information, authorization statement, a signature line, and a space at the end to attach a voided check or a deposit slip.

This form is very similar to the second form template. The only difference is that I changed the table colors from white to black, and the title text is now white to make it stand out.
Other than that, the form is exactly the same.

This form is complete, as it contains all the necessary information. I have also increased the size of the Authorization Clause or Authorization Statement and outlined additional conditions. You can fully edit these conditions as needed.
