Are you looking for a Costco Cake Order form that you can fill out online?

To order a custom cake from Costco, you’ll need to fill out a form at least one day in advance.

This form includes all the details about the cake and the design elements.

Unfortunately, as of now, Costco does not offer an online order form. You have to visit the Costco Bakery section in person, pick up the form, fill it out, and hand it over to them a day ahead of time.

Since no one was offering an online form, I decided to create one myself.

In this article, I’ll be sharing two forms: the Special Order Cake Form and the Sheet Cake Order Form.

How to use the forms

Simply click on the “Get this template” button below the preview image, then click on the “USE TEMPLATE” button to make a copy for yourself.

You can fill out the form online and print it, or simply print the form as is and fill it out by hand.

Please note that the form is designed in Google Docs, so you need to have a Google account to make a copy.
