Before I share the templates, I would like to explain how you can access them. As I mentioned earlier, I have designed these templates in Google Docs, which means you need to have a Google Account to access them.

Once signed in to your Google Account, you can follow these easy steps:

I thought it would be a good idea to know all the fields that are needed in an ABA intake form. After researching, I made a list of fields that must be in an ideal ABA intake form.

I then divided the fields into sections to organize them, and then I began designing the template. Initially, I decided to just list all the fields and leave blank space for the user to write.

However, the end result was shabby and unprofessional. So, I used tables to organize, and the result was a professional-looking form.

This covers all the details like client and guardian details, emergency contacts, insurance data, medical and developmental history, behavioral concerns, communication abilities, educational background, therapy goals, and consent.

After designing the first template, I felt that the form looked too professional!

To be honest, it felt like a form I’d see in banks! I wanted to make it look a bit more modern and minimal.

I then decided that I’d keep the font the same, but I would remove the side borders of the table. Also, the table headers should have a different color, so I went with a very light shade of cyan.

This made the form look minimal and bright! If you want, you can also change the page color, the table border color, font size, font, etc.

Initially, I was just going to publish the above two templates, as they have everything an ideal ABA intake form should have.

But then I had this thought: if I were the client and the clinic handed me this form, what thoughts would I have?

The first thought was that if I were in a hurry, I probably wouldn’t fill out this long form; it’s four pages long.

So, I thought, why not design a 2-pager and a one-page form as well? And this is when I decided I would create four more forms.

Two would have the exact same content, but I would make the other twp templates look minimal, just like the second template.

This is how Templates 3, 4, 5, and 6 were born.

Above is the 2-page template that I came up with. It has all the core details like demographics, emergency contact, diagnosis, medical history, behavior concerns, communication abilities, and therapy goals.

And here is the 2-page template, which has the exact same content as the third template, but a different design.

I made it look minimal, just like the second template.


5. One-Page ABA Intake Form Template

This is the third type of template that is ideal when a client is in a hurry. I again decided to make two versions: a black and white one, and a color one.

This form includes key client details, medical and diagnosis info, behavior concerns, communication methods, and therapy goals.


6. One-Page Minimalist ABA Intake Form Template

And here is the one-page minimalist form. I have again used a very light shade of cyan. You can totally change this color to any color of your choice.

I would suggest pastel colors.

After designing the above 6 templates, I had this idea of designing forms for our youngest clients. Parents would want forms that would truly speak to their child’s specific journey.

8. Insurance Pre-Authorization ABA Form Template

This one is a bit unique, and you will see no one else is offering such a template on the internet.

This form was designed to streamline the entire insurance pre-authorization process. I have included all the necessary insurance plan data.

This is a hybrid form where you are basically asking the client to fill it out, but then there might be some initial observations and assessments which you might want to jot down.

That’s why I have added three important sections to the form: INITIAL OBSERVATIONS, THERAPIST NOTES, and IMMEDIATE PLAN.

Sometimes, a lot of text can be overwhelming, so I decided why not make a simple form, use icons to enhance understanding, which will make filling the form easier and more fun for the client.

I have designed two forms, and this one is a basic form with no colors.

And the last template is the most beautiful-looking ABA intake form you’ll ever see.

The content is the same as the previous form; it’s just that I have changed the color, right from the page color to the text, table border, and table background.

I have also divided each field into a different table to make it look cleaner.
