Other websites that offer caregiver forms don’t provide them for free, or they offer a random form that you can fill out for free, but when you try to print the form or export it as a PDF, you’re either asked to pay for it or sign up with your email.

But if you want all the forms, the ability to edit them on your own, on a trusted platform, and all for free and without any registration, then you’re in the right place!

In this article, I will be sharing these 11 essential forms that will make caregiving super easy and organized:

How to Use These Forms

Using these forms is super easy!

All you need to do is find the form you want to use from this article, then preview the form.

Below the preview images, you will see the “Get this template” button. Just click on it and then click on “USE TEMPLATE” to save a copy for yourself. You can then start editing.

The form will be stored in your Google Drive. Once you’re done making a copy, you can either print it and fill it out offline, or fill in the form and then print it. You can also export the edited form as a PDF and share it.

1. Caregiver Agreement Form

Before you hire a caregiver, you need to have a formal agreement with them. This formal agreement must include all the details about the service, from duties and schedule to employment status and reimbursement.

This free caregiver agreement form template covers all the essential terms you need. Instead of writing everything from scratch, I’ve provided a helpful template to get you started.

You can use the form as is, or you can add more details or terms if I’ve missed anything. Since it’s created in Google Docs, you can fully edit the form.

In the template, I’ve included blank lines where you should add the information. Please note that you need to delete these lines before entering any text. For added clarity, you can bold or underline the information by selecting the text and then clicking the Bold or Underline feature in the toolbar.

2. Caregiver Checklist

The caregiver checklist template is a simple tool that allows caregivers to track daily tasks. In this template, I’ve included a space at the beginning to enter the caregiver’s name, followed by the date and time when the caregiver started working, as well as the end date, in case you have multiple caregivers working different shifts.

I’ve divided the tasks into five sections: personal hygiene, nutrition, household chores, health monitoring, and daily activities. I’ve added sample tasks in each section, but you can edit these tasks and even add more as needed. If you want more rows in any section, simply right-click and select “Insert row below” to create a new row.

Next, I’ve included a “Status” column with drop-down options. You can click on the drop-down and choose the appropriate status for each task. Additionally, I’ve added a column for timing and a special column for notes.

3. Caregiver Daily Task Sheet

This is another simple template that allows you to track daily tasks. I know this template may look similar to the one above, so you can choose either this one or the previous template depending on your preference.

In this template, I’ve divided the tasks into four sections: Morning, Afternoon, Evening, and Overnight. I’ve also included a special table to track daily observations and add notes.

You’ll notice that I haven’t included a drop-down in the “Completed” column like in the earlier template. Instead, I’ve left the column blank.

To mark a task as completed, you can simply insert a checkmark next to the task. You can either copy the checkmark symbol from the column title, or if you want a different style of checkmark, go to Insert > Special character. In the search box, type “check,” and choose the checkmark style you prefer.

4. Caregiver Daily Cleaning Log

This template allows you to list and track the daily cleaning tasks for any room, from the living room to the kitchen. You can keep all your cleaning tasks organized in one place.

For each task, you can specify the cleaning area (e.g., bedroom, living room, bathroom), describe the task that needs to be done, provide details for that task, and include any special notes.

There’s also a column where you can insert a checkmark to indicate when a particular task has been completed.

Additionally, I’ve added a table where you can add any extra tasks, and another table below it for notes.

5. Caregiver Medical Administration Record

This is a super important template that allows you to keep track of all the medications required for the patient.

Firstly, I’ve created a section where you can enter the date, the caregiver’s name, and the patient’s name. Next, you’ll find basic medication details such as the name of the medicine, dosage, route, purpose, and the prescribing doctor.

I’ve also included an administration log where you can track which medicine was given at what time. Below that is a small table for logging notes or side effects, if any, and finally, a weekly overview of the medications.

6. Blood Pressure Log

This simple template allows you to log blood pressure readings throughout the day and week. The first table includes columns for systolic, diastolic, and pulse measurements, as well as the position of the patient during the reading.

Next, there is a weekly overview table where you can enter the average readings, followed by another table where you can list observations and notes.

7. Blood Sugar Log

Just like the blood pressure log, I’ve created a template that allows you to log blood sugar levels throughout the day and summarize the average readings, along with noting any observations.

If needed, you can convert this template into any other log that you may require.

8. Pain Tracker

The patient may experience pain in different parts of the body. This template allows you to track those pains.

You can list the location of the pain, enter the medication used, record the pain intensity (on a 1-10 scale), note symptoms, activities performed before the pain, and medications. There is also a special column to track the effectiveness of the medication given (on a 1-10 scale).

Additionally, I’ve included a dedicated section for recording pain relief methods, timings, and comments. I’ve also provided a table for a weekly overview of the pain and another table for weekly analysis of the pain.

9. Wound Care Tracker

The wound care tracker allows you to monitor and document a wound.

You can track details such as the wound’s location, type, size, and depth, as well as daily assessments like drainage type, odor, appearance of the wound bed, and the condition of the surrounding skin.

Additionally, there is a wound treatment log, a weekly wound overview, and a weekly summary.

10. Elderly Care Sheet

I wanted to create a single document where you can track everything. Instead of maintaining multiple trackers, you can use this template, which covers all the essential aspects of daily care.

This free template includes sections for monitoring toilet use and diaper changes, meals and fluid intake, mood, medication administration, blood pressure, pain levels, and more.

I’ve prefilled it with checkmarks. Please remember to remove them before printing or making any edits.

11. Simple Caregiver Form

This is a simple and minimal caregiver form where you can track daily care activities, medication, meals, and special care instructions.

I’ve included sections where you can record meal times and details, activities, sleep, personal care tasks like bathing, dressing, and grooming, as well as medication dosages and times.

This template was designed for offline use, allowing you to simply print it and fill it out by hand. However, if you prefer, you can also edit it in Google Docs and then print it.
